Providing the Care Kids Need to Thrive

The Perkins Foundation is dedicated to supporting children and young adults (ages 1–20) by helping cover the costs of medical care, dental services, eyeglasses, and hearing aids. While our resources are limited, we strive to assist as many families as possible, focusing on those with demonstrated financial need. Applying is simple—complete the application form below or stop by our office at 45 East Loucks, Suite 110. Please include a copy of your most recent Federal Tax Return, and if you haven’t filed, just let us know. We’re here to help your family get the care you need.

Guidelines:

Applicants must have lived in Sheridan County for at least one year and be between the ages of 1 and 20. The application and all supporting documents must be completed by the applicant, parent, or guardian. Applicants must demonstrate financial need. All applications require board approval before any medical or dental care can be provided. The B.F. and Rose H. Perkins Foundation has the final authority to determine eligibility and approve grants.
